The SAR (Safeguarding Adults Review) referral form was approved by Dudley Safeguarding Adults Board (DSAB) on Friday 2nd February 2018. Please find the attached SAR referral form along with the SAR pathway and SAR policy for your convenience.
The Care Act 2014 requires Safeguarding Adults Boards to arrange Safeguarding Adults Reviews (previously known as Serious Case Reviews) if there are concerns that agencies could have worked more effectively to protect an adult from serious harm or abuse; whether the adult has died or not.
The purpose of a Safeguarding Adults review is not to re-investigate or apportion blame but to establish whether lessons can be learnt from the circumstances of a case that may improve practice or the way in which agencies and professionals work together to safeguard vulnerable adults.
